NHS Consultation on Pharmacies

The NHS in Dartford, Gravesham & Swanley is looking at how primary care teams work together more successfully in the future to provide better local care. We would appreciate it if you would take a few minutes to provide feedback on your current experience of services and how they work together now. In this way we can measure any changes and improvements for you the patient in the future as new ways of working are put into place and developed.
